Sutter Health Plan has revised the 2025 Evidence of Coverage (EOC) documents for employer groups currently on a 2025 plan design to reflect recent changes.
They have added information relating to our new behavioral health plan partner effective January 1, 2026, Carelon Behavioral Health of California, Inc.
They also added coverage for special footwear and orthotics at the current orthotic benefit cost share for all small group plans, effective January 1, 2026.
Please review the enclosed Amendments to the 2025 EOCs for small group standard and Plus plans, as well as large group plans.
Sutter Health Plan is providing these Amendments to applicable employer groups by email. The amended EOCs are also available to employers and subscribers on their portal accounts. Registration required.
