Rates summary1:
  • Medical: Q3 2025 small business rates will have a 2.3% increase for HMO plans, and a 1.5% increase for PPO plans.1
  • Dental: No rate or plan changes at this time.
  • Vision: No rate or plan changes at this time.
  • Optional Chiro: All 2025 plans have the chiro rider option. The chiro rider is an additional $3.00 PMPM for all HMO and PPO plans.

    • Health Net's Enhanced Choice offers small business groups a simplified package offering access to all plans with no limits to the number of plans and networks you can offer
    • Chiropractic rider applies to both HMO and PPO
  • Flexible choices: Two separate underwriting promotions
  • Straightforward enrollment:
    • No DE9C, payroll or ownership documents required for groups that qualify for Health Net’s underwriting promotions
    • Health Net can be written alongside any carrier – enrolling on the other carrier is a valid waiver when enrolling at least 5 subscribers with Health Net
    • Mix and Match any of the Health Net plans and networks. No limit!
    • 2 weeks of payroll for a Startup Group of any size
    • Health Net accepts PEO subgroups with a quarterly wage report or 2 weeks of payroll from the PEO
    • Enhanced Choice: Groups with a minimum of 5 enrolling employees - mix and match any plans from the HMO and PPO networks: 
      • NO DE 9C, payroll or ownership documents
      • NO Prior carrier bill 
      • All eligible employees declining coverage must provide a waiver including those enrolling with another carrier 
      • Minimum participation: 5-100 enrolling employees, 25% participation is required
    • Sell HMO with 6 Groups with a minimum of 6 enrolling employees – Mix and match any plans from the HMO networks: 
      • NO DE 9C, payroll or ownership documents 
      • NO Participation attestation NO Prior carrier bill 
      • NO Waivers (when not paired with Life or Employer Paid Dental/Vision.) 
      • NO Participation requirement
